Patient Portal: Getting Started

What is the Patient Portal?

The Patient Portal gives patients the ability to access their personal health information at their own convenience, through a secure website online. This will help you be more actively involved in your healthcare, can help you communicate with your providers, and make you more informed.

The patient portal is currently only available for patients who have been admitted to the hospital, as a patient in the emergency room, and for outpatient therapy or lab work.

What information can I see?

In the portal, you will have access to your own electronic medical record. This will show different visits that you have had at Lincoln County Hospital.

For each visit to our facility, you will have a Summary of Care Document. This document summarizes the care that you received while you were a patient in the hospital.

Different information included in the Summary of Care Document includes:
  • Medications prescribed
  • Allergies to any medications
  • Immunizations given
  • Lab test results
  • Discharge instructions

After each visit or admission to the hospital or emergency department, you can go online and login to your Patient Portal to view a new Summary of Care Document for each visit. These documents can be accessed any time after you are discharged from the facility.

How do I setup my account?

When registering as a patient at the hospital, our registration staff will ask you for your email address, which will allow our system to send you an email invitation containing a link to your Patient Portal.

You may also call the registration department at LCH to provide or update your email address at
(785) 524-4403.
